Private Sickness Certificate*

This type of documentation is typically used for informal records of sickness. This service is not for the provision of Statutory Med 3 forms - these can only be issued by suitably qualified doctors (like GPs).

We are only able to complete a letter for acute physical illnesses, including:

• chest infections

• cold and flu viruses

• COVID-19 (upon evidence of a positive test)

• complicated urinary tract infections

• shingles

• minor injuries

Legally, there is no obligation for an employer or institution to accept this letter as proof of illness. Only Statutory Med 3 forms must be accepted by employers, and we recommend you see your GP for this.

As above, there is no way to compel an employer or institution to accept this letter as proof of illness.

The best way is for you to ask them directly what exactly they will accept as proof of illness.

A sickness certificate costs £40.

We offer a 10 minute video consultation so you can speak with a pharmacist about your illness, any specifics which your employer or institution have requested should be included, and our recommendations for you.

While this consultation is with a clinical member of staff, you will not receive in-depth clinical advice or examination during this consultation.

No. If we see you for another service you can request an add-on letter of treatment confirmation for £25 - this is a letter confirming how and why you were treated, but does not include employer- or institution- based information.
